A performance appraisal is a process for individual employees and those concerned with the individuals performance. The manager will arrange a meeting to discuss and evaluate their performance, during this meeting an appraisals needs to be constructive with developmental discussion between all parties. Managers should focus on supporting the individual to help them reach their potential. In order for performance appraisal to be effective there needs to be ongoing conversations and regular meetings that will contribute to the performance appraisal review, rather than waiting for an annual appraisal meeting.

Below is an example of a Performance Appraisal Form, this can be adapted accordingly.

What was the key learning? How did this impact on the job and how have you implemented this into the environment/practice?

2. List training, development and learning goals for next year. Identify the learning goals first and

then consider the different ways in which this learning may be achieved.
